Tdap

Dear Parent/Guardian:

An important change has taken place in the immunization requirements for students entering 6th grade.  Any child born on or after 1/1/94, and entering 6th grade on or after September 1, 2007, will be required to show proof of a booster immunization containing diphtheria and tetanus toxoids and an acellular pertussis vaccine, which is commonly known as Tdap or Adacel.

This is not the booster your child received as a 5 year old.

This proof must be received in the nurse’s office before the student can begin 6th grade.  The immunization can not be given until your child is 11 years old.  If they do not turn 11 until after the start of the school year, they will be required to have the immunization within two weeks of turning 11 years old to continue in school.

In order to comply with the New York State regulations, the following suggestions are offered:

1.     Make arrangements with your child’s physician to have the immunization, and send proof of immunization to the nurse’s office.

2.     Take your child to the Oneida County Health Clinic, located at 300 W. Dominick St., Rome, NY (336-9217).  The clinic is open Tuesday from 1:00 to 3:00 pm.  There are also evening clinics from 4:30 to 6:00 pm on the second Tuesday of each month.  After getting the immunization, be sure that you send the proof of immunization to the nurse’s office.

If your child has already received this immunization, please send in a copy to the nurse’s office.  If you do not have proof of immunization, I have enclosed a release of information form.  If you sign and return the form I can obtain the information from your Doctor.
